About the Patient Reference Group
As part of an on-going Department of Health initiative, we are hoping to expand our Patient Reference Group.
We will use this group to gain the thoughts and ideas of our patients in order to help us, and to involve you, in planning new or changing services.
We hope to be able to enrol as many patients as possible in order to ensure that the group is representative of our population. We are especially interested to hear from you if you feel you are able to represent a specific sector of our patient population.

For example:
- Carers
- Families with young children
- People from black and ethnic minorities
- People with learning disabilities or their families
- People with physical disabilities or their families
- Commuters and shift workers
- Single parent families
This list is not exhaustive. If you feel you would be able to contribute, please feel free to join the group.
In order to run the group efficiently and effectively, we are hoping to start with a virtual reference group contacting you by email or post.
If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.
We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.
Many thanks for your assistance.
